Community Health Workers (CHWs) act as trusted liaisons between communities and healthcare systems, providing essential outreach, health education, patient navigation, and informal counseling. They improve health outcomes by conducting home visits, advocating for patient needs, managing care transitions, and promoting preventative health practices
Key Responsibilities and Duties
- Outreach & Education:Conduct home visits and organize community meetings to provide education on hygiene, nutrition, maternal/child health, and disease prevention
- Care Coordination:Assist patients in navigating health systems, making appointments, securing transportation, and facilitating referrals to medical or social services
- Advocacy & Support:Serve as a community advocate, offering emotional support, health coaching, and advocacy for marginalized populations
- Health Screenings:Provide basic health check-ups (e.g., blood pressure checks, tracking child growth) and distribute essential health items.
- Data Collection:Collect community feedback and maintain documentation to inform future health programs and policies
